Designer can't load content when project has multiple content references

When a project references more than one content project, the designer can't load assets like fonts and textures.
Workaround by putting all content in one project.

The problem only occurs when more than one content project has the same Content Root Directory. The designer looks at each content project, determines its output directory, and then constructs a path to the XNB file it wants in that directory. If the file exists, then it is added to a list of possible file names. To succeed, there must be only one file on the list. However, when more than one project has the same output directory, then the list will contain duplicates, and will cause a failure. The fix is to prevent duplicates from being added.
